Fangobalneoterapia sulfurea e variazioni della pressione arteriosa: studio osservazionale

Riassunto

Premessa e Obiettivo
Attualmente le reumoartropatie risultano essere una delle principali cause di disabilità con negative ripercussioni sia sullo stato di salute della popolazione e sia sulla spesa sanitaria. In tale ambito la terapia termale fango-balneoterapica (FBT) può indurre benefici effetti. Da tempo però si dibatte sull’ipotesi che tale tipo di terapia possa incidere negativamente sulla funzionalità cardio-vascolare, il che spesso ha indotto prudenza, se non addirittura controindicazione, all’uso di tale trattamento in pazienti sofferenti di alterazioni cardio-vascolari, come l’ipertensione arteriosa, oltre che di reumoartropatie. Scopo del presente studio osservazionale quindi è stato quello di valutare, in soggetti artroreumatici, gli effetti di un ciclo sulfureo FBT sulla pressione arteriosa e la eventuale comparsa di reazioni avverse.


Pazienti e Metodi

Sono stati osservati 169 soggetti, di età compresa tra i 42 e gli 86 anni, sofferenti di malattie artroreumatiche che effettuavano un ciclo termale di FBT con acqua minerale sulfurea per 2 settimane. In base ai valori pressori arteriosi rilevati prima dell’inizio del trattamento termale sulfureo, i soggetti esaminati sono stati suddivisi in tre gruppi: NORMOTESI (NOR= soggetti con valori pressori entro il range di normalità); IPERTESI (soggetti con valori pressori al di sopra del range di normalità; questi ultimi a loro volta sono stati distinti in IPET, ossia ipertesi in terapia farmacologica con antiipertensivi ed IPENT, ossia ipertesi non in terapia farmacologica antiipertensiva). I valori pressori arteriosi, massimi e minimi, espressi in mmHg, sono stati rilevati in 1° (T1) – 6° (T6) e 12° (T12) giorno di cura. Indi sono stati comparati i valori pressori arteriosi medi±DS rilevati: a) pre e post seduta FBT sulfurea nel 1°-6° e 12° giorno di cura; b) post-ciclo FBT sulfureo vs basale (ossia pre-ciclo FBT sulfureo). La valutazione statistica è stata eseguita con test t di Student per dati appaiati. Valori di p≤0,05 sono stati considerati statisticamente significativi.


Risultati
La valutazione comparativa dei valori rilevati pre-seduta FBT sulfurea vs post-seduta FBT sulfurea nel 1°-6° e 12° giorno di cura ha evidenziato nei soggetti artroreumatici ipertesi, in terapia farmacologica antiipertensiva e non (gruppi IPET e IPENT), una significativa (p<0,05) riduzione dei valori medi pressori arteriosi; mentre nei soggetti normotesi (gruppo NOR) le riduzioni pressorie sono risultate modeste e al limite della significatività statistica. Analogamente la comparazione dei dati rilevati a fine ciclo FBT sulfureo (post-T12) vs basale (pre-T1) ha mostrato: nei gruppi IPET ed IPENT una significativa (p<0,01) diminuzione dei valori medi pressori arteriosi, sistolici e diastolici; mentre nel gruppo NOR la riduzione è risultata di minore entità e statisticamente significativa (p<0,05) solo per i valori medi arteriosi sistolici. Una buona tollerabilità locale e sistemica del trattamento FBT considerato è stata anche osservata.


Conclusioni
I risultati del nostro studio, in accordo con i pochi dati di letteratura, evidenziano la possibilità di inserire il ciclo termale FBT sulfureo nel protocollo terapeutico interdisciplinare di soggetti in cui alla malattia artroreumatica si associa anche ipertensione arteriosa

 

 

 

Abstract

Sulphureous mud-bath therapy and changes in blood pressure: observational investigation


Background and Objective
The chronic arthropathies currently appear to be a major cause of disability with a negative impact on quality of life and health care spending. The mud-bath therapy is a spa treatment that induces benefic effects in chronic rheumatic diseases. It has long been debated on the assumption that the mud-bath spa therapy could have adverse cardiovascular effects which often induce caution and even a contraindication to the use of this treatment in chronic arthropathies associated with cardiovascular alterations such as hypertension. The aim of this observational study was to investigate, in arthrorheumatic subjects, the effects of sulphureous mud-bath cycle on blood pressure and the possible appearance of adverse drug reaction.


Patients and Methods
169 patients, with age range 42-86 years, suffering by chronic arthropathies were treated with sulphureous mud-bath therapy for 2 weeks. According to the arterial pressure values, measured before the spa treatment, the patients considered were divided in three groups: with normal blood pressure (NOR group); with high blood pressure, after, the latter group was divided in IPET (patients in treatment with antihypertensive drugs) and IPENT (patients not in antihypertensive therapy). The arterial pressure values, maximum and minimum, expressed in mmHg, were detected in the first (T1) – sixth (T6) and twelfth (T12) day of spa treatment. The media arterial pressure values collected before and after T1, before and after T6, before and after T12 , before T1 and after T12 were compared. The data, presented as mean±SD, were compared with the paired Student t test. A p value ≤0.05 was considered significant.


Results
The comparison between the mean values detected in pre and post T1, pre and post T6, pre and post T12 have showed that sulphureous mud-bath therapy induced a significant (p<0.05) reduction of arterial blood pressure values in patients suffering of chronic arthropathies with high blood pressure in antihypertensive therapy or not (IPET and IPENT groups); while in patients with normal blood pressure (NOR group) were observed modest reduction at the limit of statistical significance. Similarly, the comparison between the data detected at the end of sulphureous mud-bath therapy (post-T12) vs baseline (pre-T1) have demonstrated: in IPET and IPENT groups a significant (p<0,01) decrease of arterial blood pressure values; in NOR group very small decrease, this reduction is significant (p<0.05) only for maximum arterial pressure value. Were not observed adverse drug reaction.


Conclusions
The results of our study, in according with the few data in the literature, evidenced that is possible include the sulphureous mud-bath therapy in interdisciplinary therapeutic protocol of patients suffering of chronic arthropathies and arterial hypertension.
